Introduction
What is this questionnaire?
This is the questionnaire for the mining site owners or mining license holders or concession holders, who have been introduced to our company, and who know what we may offer in a joint venture.
When are you to complete this form?
You will complete this form under following conditions:
That the mining site owner, or mining license holder, concession holder knows what our company is doing, and that we are engaging in joint ventures. That information is sent by email. If you don't know what we are offering, don't complete the questionnaire.
If you are intermediary, than both you as intermediary and the mining site holder must know what our company is offering. If you don't know, ask. If you have not received the emails or other information about our offer, do not complete the questionnaire.
That you wish to enter the joint venture, so that our company may invest into the mining equipment, management and organization of the mining site.
Read well the above points and if you think you satisfy all points, you should answer the questions.
Mining sites with high priority
Mining sites with high priority for our company have one or more following characteristics:
gold bearing ore have been found and is currently being processed on the mining sites, gold is being sold on daily or weekly basis. There are many shafts on the mining sites and many people working.
gold bearing ore have been found, well identified on multiple locations on the mining sites, and may be proven within shortest time, however, there is no operation on the mining site and no people working.
alluvial gold is being processed on the mining site.
mining site has the water source such as river, lake, large pond, stream, without limitations.
mining site has accommodation, offices, and equipment.
mining site is not far from water sources, it is very near the main road, electricity and villages with necessary resources.
there is mining licences so that mining operation may start sooner.
there is accurate geological report of the underground minerals.
there is environmental plan and all the administration in place.
mining site holder have capital for the joint venture.
Mining sites with low priority
there was no prospecting on the mining site in past. Prospecting is maybe asked for.
there is no mining license, just a prospecting license.
geological reports are wishy-wash.
environmental plan is missing.
administration is not in place, and there is much yet to be done.
no gold has been determined yet on the mining site.
no equipment, no accommodation.
mining site holder has no capital for joint venture.

This questionnaire is to be completed for each separate licenses land, or alternatively for the group of the adjacent licensed lands together. It is required before we can propose you the next step in development of your mining site.

[ ] What was the main reason that you have decided to obtain these license(s)? Please be very descriptive. Why did you choose this position and obtained licenses on that location?

[ ] Did you conduct the practical and specific underground research of minerals in order to make the geological report?
[ ] What is the name of the geologist who maybe participated in the geology report? Provide his phone number please and email address if any, and the postal address.
[ ] What is your current investment amount into administration, obtaining licenses, travel, geological report, equipment? Please be specific, list your investment amounts.
[ ] What type of license is it? Is it Prospecting License or Primary Mining License, or which type is it?
[ ] Tell us the first year, when the license was paid?
[ ] Until what year is that license valid?
[ ] Did you send us the copy of your mining license? We need to receive the copy of the mining license to make sure that facts are valid and to observe the mining site on the satellite map.
[ ] Did you have a Prospecting License in past for this land?
[ ] Did you prospect on this land? Describe also the tools and methods you used to prospect on this land.
[ ] Location of the mining land, describe it, where it is? What towns are nearby?
[ ] Do you know the land size?
[ ] Is there or was there any agreement for joint venture or for the usage of this land previously? Even if there was a previous verbal agreement with artisanal miners, please describe it here.
[ ] Do you expect to have alluvial gold on this land? And also describe, why do you expect to have alluvial gold? What indications are bringing you to say so?
[ ] Do you expect to have mother lode gold or gold in rocks on this land? And also describe why do you expect to have gold in rocks?
[ ] Are there any people working currently on that mining land?
[ ] Did you have people in past working on that land?
[ ] How many people?
[ ] How much gold is extracted per month from that mining land currently? If you don't know exactly, provide some sample sales and times.
[ ] Or do you know how much gold do you get per bag of material?
[ ] How many tonnes or how much material do you process every day? Or how many bags, buckets?
[ ] Do you use mercury on your mining site? Was mercury ever used before?
[ ] If you are not using mercury on your site, do you use mercury on some neighboring sites?
[ ] How large is mining activity on your site?
[ ] What equipment do you already have on mining site? What is the capacity of such equipment? How much ores or material can you process?
[ ] Who invested moneys into the equipment? Was it you? And how much did you invest in equipment?
[ ] Is there accommodation on the mining site? And what type of accommodation is it? For how many people?
[ ] What type of soil or ores is there, is it hard rocks, soft ores? gravel, sand, rocks, clay?
[ ] What minerals are available on the land? Do you know the names of minerals containing gold, that are available on your mining site?
[ ] What type of mining is available on that mining land? Is it just traditional mining, or you have some modern operation?
[ ] Tell us about the shape of the land? Does it have hills, big rocks? Is the land flat?
[ ] Are there other mining sites available nearby?
[ ] How far is the next main road?
[ ] How far is the next electricity line?
[ ] Is there any water source, such as pond, stream, river, well, nearby, that you know? Is it on your site? Do you use that water source for mining purposes?
[ ] How far is the water source?
[ ] Name of nearest larger town and its distance?
[ ] What is most needed that mining activities start on that mining land?
[ ] Do you have your own business plan for development of your mining site?
[ ] Do you have your own capital? Are you willing to invest in the gold mining business and what is your proposed or available amount of capital?
[ ] If you have your own capital, would you like that we manage the mining site for you?
[ ] Are you aware that we are proposing fair profit sharing, that means, how much you invested, that is how much you would receive in profits. Your ratio of investment, shall be equal to ratio of profits you are receiving. If you have other proposals, let us know, and tell us now and here.
[ ] Are you willing to have foreign or local partner, to partner with you, so that your mining site can be developed to production of gold, and that profits can be shared?
